General comments

Algemene opmerkingen:

- Necrotiserende weke delen infecties kunnen monomicrobieel zijn (dan meestal Groep A streptokokken) of polymicrobieel (mengflora met onder andere anaerobe verwekkers).

- Invasieve groep A streptokokkeninfectie is een meldingsplichtige ziekte groep B2; Gezinscontacten meebehandelen en andere nauwe contacten controleren (zie LCI-richtlijn: Groep A-streptokokkeninfectie [GAS]).

- Bij (verdenking op) fasciitis necroticans zo spoedig mogelijk overleg met chirurg en kinderinfectioloog.

 

Algemene therapeutische principes:

- Spoedindicatie voor chirurgische behandeling

- Bij sepsis gentamicine toevoegen (https://www.kinderformularium.nl/geneesmiddel/23/gentamicine)

- Geef immunoglobuline intraveneus, eerste dag 1 g/kg, dag 2 en 3 0,5 g/kg, totaal 3 dagen.

- Antibiotica, voor zover mogelijk, stroomlijnen op basis van de kweekuitslagen.
